There are a number of form modules on the market that work with DotNetNuke, each with their own strengths and weaknesses depending on what you need to get done with them: general information gathering, work flow management, payments processing, and even integration with other systems. Some are oriented more toward being configured by someone with a technical aptitude, while others can be set up and managed by non-technical users.
